Patient Forms

If you are preparing to visit Desert Dental Group Children’s Center for the first time, or if you are bringing in a new patient family member, we ask that you please fill out the forms attached here. These new patient forms are used so that we can get an understanding of your medical history and past dental history. By filling these forms out now and bringing them with you on your appointment, you’ll save time having to do them once you get to the office. It makes your appointment that much faster!
