Important Details To Consider
- The membership plan runs for 12 months and automatically renews. You can opt out of the auto-renewal at any time using the Member Portal.
- To receive the benefits of the membership plan, your payments must be current.
- The membership plan provides coverage for a range of healthcare services and products. However, any treatment not explicitly included in the plan will require payment at the time of service.
- The membership plan cannot be combined with insurance plans or other offers or discounts.
- For specialist treatments outside our practice, the membership plan may not apply.
